Should you need a note or a revised note for school, daycare or your employer when your child is out sick, please email us with the following information: Our patient name, date of birth, days you would like the note to cover, your name and phone number, and your fax number, or the fax number to the school, daycare or employer. We will complete your request or call you if there is a problem.
