Thank you for your trust and confidence in allowing me to care for your children for the past 15 years. It has been an honor and privilege for me, so it is with mixed emotions that I announce that effective September 4, 2022, I will no longer be practicing at ABCD Pediatrics.
You can continue to receive excellent healthcare from any one of my colleagues at the Stone Oak office, including Dr. Janae Maher and Dr. Eesha Farooqi.
ABCD Pediatrics will remain as the custodian of your medical records, and your records can only be released or transferred with your permission. If needed, an Authorization to Release or Disclose Form is available at any of our offices, or on our website under downloadable forms.
