How does PCMH (Patient Centered Medical Home) relate to my child’s care?

PCMH is a model designed to improve health care in America by changing how your primary care clinic is organized and delivers care. Our locations at Stone Oak, Schertz, New Braunfels and Bulverde have been awarded recognition by the NCQA Patient-Centered Medical Home Program. Our new location in Boerne is currently undergoing transformation to PCMH Recognition.

The PCMH model encompasses five functions and attributes:

Comprehensive Care: ABCD Pediatrics is accountable for meeting the large majority of each patient’s physical and mental health care needs, including prevention and wellness as well as acute and chronic medical issues. Since we do not have all the available specialties to meet some patient’s complex needs, we align our practice with those that can.

Patient Centered: ABCD Pediatrics medical home provides health care that is relationship-based with an orientation toward the whole person. Partnering with our patients and their families requires understanding and respect to our patient’s unique needs, culture, values, and preferences. ABCD Pediatrics actively supports patients in learning to manage and organize their own care at the level the patient chooses. Recognizing that patients and families are core members of the care team, we ensure that you are fully informed partners in establishing care plans.

Coordinated Care: ABCD Pediatrics coordinates care across all elements of the broader health care system, including specialty care, hospitals, home health care, and community services and supports. Such coordination is particularly critical during transitions between sites of care, such as when you are being discharged from the hospital. At ABCD Pediatrics we strive to build clear and open communication among patients and families, the medical home, and subspecialists.

Accessible Services: ABCD Pediatrics delivers accessible services with shorter waiting times for urgent needs, enhanced in-person hours, around-the-clock telephone or electronic access to a member of the care team, and alternative methods of communication such as portal access and follow-up telephone care. We do our best to place patients’ preferences regarding access.

Quality and Safety: ABCD Pediatrics demonstrates a commitment to quality and quality improvement by ongoing engagement in activities such as using evidence-based medicine and clinical decision-support tools to guide shared decision making with patients and families, engaging in performance measurement and improvement, measuring and responding to patient experiences and patient satisfaction, and practicing population health management. Sharing robust quality and safety data and improvement activities publicly is also an important marker of a system-level commitment to quality.

What is Patient Centered Medical Home?

Our locations at Stone Oak, Schertz, New Braunfels and Bulverde have been awarded recognition by the NCQA Patient-Centered Medical Home Program. Our newest location in Boerne is currently undergoing transformation to PCMH Recognition. As such, we strive to be the “Medical Home” for your children and provide equally for all our patients a safe environment where you can expect to receive a comprehensive assessment and a personalized treatment strategy based on the most current evidence based guidelines. We will conduct surveys from time to time, and ask that you answer thoughtfully as we will use your feedback to guide us toward quality improvement that is important to you.

Please think of us as the first place to come for advice and care regarding all your child’s health care needs. This includes physical, developmental and emotional health, as well as a place to help you find the appropriate specialist or access community resources. We also want to coordinate your care. Let us know if you have been treated by a specialist, an emergency department or any outside healthcare professional. Informing us of medications, test results, and procedures are keys to having a complete understanding of your child’s health. Every time you are seen for care outside of our office, please request that a copy of the visit or test result is sent/faxed to us so that we can best serve you.

In addition, let us know if you are using any over-the-counter medications or have undertaken any self-care regimens. Review your patient information on the patient portal and tell us if the information we have on file is incomplete or inaccurate. Good care requires a partnership between your family and our office. We appreciated your help and look forward to working with you.

Benefits For Patients

  • Stay healthy. Patients who are treated in PCMHs tend to receive preventive services and screenings at a higher rate than patients who are not in PCMHs, helping them stay healthy.
  • Better communication. Communication with patients and their families/caregivers is a core concept of the PCMH model, which also emphasizes enhanced patient access to clinical advice and medical records.
  • Better management of chronic conditions. According to research, PCMHs are especially helpful for patients with complex chronic conditions.
