Sports Physicals
Sports Physical season is upon us, and we are striving to simplify the process for you. Parents: please be aware that we have noticed some school districts have now placed a date of when they will accept a signature on your Sports Clearance Forms. Please note this date when scheduling or dropping off your forms. We also ask that you allow us 3-5 business days to complete these forms. Your child may require an additional appointment depending on when the last time he/she was seen.
